Correction Notice for Ezetimibe with Simvastatin monographs in AHFS DI and AHFS DI Essentials

Please note three corrections to typographical errors in the AHFS Drug Information 2006 (AHFS DI 2006) and AHFS DI Essentials 2005-2006 monographs for Ezetimibe 24:06.05 and Simvastatin 24:06.08. Full, correct monographs are available at http://www.ashp.org/ahfs/notices.cfm .

In the first column on page 1659 and in the second column on page 1698 of the printed edition of AHFS DI 2006 under "Preparations," the last entry listed for Vytorin should read: "10 mg Ezetimibe with Simvastatin 80 mg."

Likewise, in the second column on page 1703 of AHFS DI Essentials 2005-2006 under "Preparations," the last entry listed for Vytorin should read: "10 mg Ezetimibe with Simvastatin 80 mg."

The current description of this formulation of Vytorin reflects a typographical error in the strength of the combination and is incorrect.

The editors of AHFS DI and AHFS DI Essentials urge you to correct the errors in all copies of AHFS DI and Essentials and to communicate to others on your staff who may use these books. A notation correcting this error should be made in the margin of the affected pages.

Electronic subscribers to AHFS DI and Essentials data will receive replacement files incorporating these corrections in the next scheduled update. In the interim, electronic subscribers should consult and bookmark http://www.ahfsdruginformation.com .

In all monographs, the text under "Dosage" that describes recommended dosages of ezetimibe and simvastatin when administered as the fixed combination Vytorin is correct as published. Other entries that list the ingredients of Vytorin (i.e., those containing 10 mg Ezetimibe with 10, 20, or 40 mg Simvastatin) also are correct.

Notices have been sent to all subscribers of AHFS DI and Essentials as well as to directors of hospital pharmacies. In addition, notices have been posted in a variety of portions of the American Society of Health-System Pharmacists Web site, http://www.ashp.org .

The error also appears in the 2005 edition of AHFS DI, which is out of print and should not be relied on for clinical decisions due to the changing nature of drug information.
