Textbook and Publication Errata
Lexicomp's Pediatric Dosage Handbook,
10th ed. contains ambiguous reconstitution directions for
palivizumab
On page 853, under the palivizumab monograph, it states:
"Reconstitute single-use vial with 1 mL of SWI".
Since there are 2 strengths of palivizumab available in single
dose vials, 50 mg and 100 mg, with different amounts of diluent
required for each, this is a potentially confusing reference
which can lead to dosing errors.
Lexicomp has been contacted and will change future references
to state:
"Reconstitute by adding 0.6 mL of SWI to each 50 mg
vial or by adding 1 mL of SWI to each 100 mg vial"
