Our plans are ambitious, but our dedicated staff are committed to strengthening our ability to help you prevent medication errors.

Key Accomplishments in 2011

  • Identified best practices with the timing of medication administration, which led to changes in the CMS “30-minute rule” for timely drug administration
  • Completed the 2011 ISMP Medication Safety Self Assessment for Hospitals study—more than 1,300 hospitals participated
  • Released the results of pivotal surveys showing the impact of drug shortages and gray market activity on patient safety, which helped inform an FDA summit on the issue; federal investigation of price gouging; ISMP participation in meetings with the Secretary of Health and Human Services, FDA Commissioner, and House and Senate committees; and an executive order from President Obama to respond to the drug shortage crisis 
  • Conducted an IV Safety Summit focused on pharmacy compounding of sterile products
  • Conducted a collaborative with hospitals on the Safe Use of HYDROmorphone 
  • Collaborated with Vermont Oxford Network and NICUs to develop Standard Concentrations of Neonatal Drug Infusions
  • Launched a Readiness Assessment for Barcode Verification Systems in Community Pharmacies
  • Received approval for continued listing as a Patient Safety Organization (PSO)

Key Plans for 2012

  • Launch the 2012 ISMP International Medication Safety Self Assessment for Oncology (January)
  • Develop a High-Alert Medication Modeling and Error Reduction Scorecards (HAMMERS) tool for community pharmacies
  • Complete a 4-year study on patient counseling when high-alert medications are dispensed from community pharmacies
  • Develop a Root Cause Analysis Workbook for Community Pharmacies
  • Establish an ISMP Fellowship for Physicians in collaboration with a local teaching hospital
  • Launch an insulin safety component on www.ConsumerMedSafety.org  
  • Redesign the www.ISMP.org website to maximize access to tools and information
  • Redesign and relaunch the www.ConsumerMedSafety.org website to maximize access to consumer tools and information
  • Begin a 4-year study as a consultant with the University of Illinois in Chicago on simulation training for opioids and drug name testing
  • Publish an updated 3rd edition of the Medication Errors book
  • Establish guidelines and tools to assist the second victims (practitioners who make errors) and third victims (organizations where errors happen) of medication errors
