Correction Notice - Pediatric Injectable Drugs, Ninth Edition

There are three new, important corrections to monographs in Pediatric Injectable Drugs, Ninth Edition (The Teddy Bear Book), published by ASHP.

 

1. In the monograph for HydrALAZINE HCl on pages 288–9, in the Dosage section, the second line for Hypertension/heart failure should read as follows:

50-100 mg/m2/day

 

2. In the monograph for Potassium Chloride on pages 474–5, in the Maximum dosage section, the second line should read as follows:

Intermittent infusions should not exceed 1 mEq/kg/hour

 

3. In the monograph for Filgrastim on pages 250–1, in the Preparation and delivery section, the second and third lines should read as follows:

normal human serum albumin should be added at a final concentration of 2 mg/mL.

 

Please make these corrections immediately in all copies of the ninth edition and communicate it to everyone who may use the book
