Using Data Effectively to Manage the Risks to Medication Safety
This one-day interactive program, being offered by the Institute for Safe Medication Practices (ISMP) and the United States Pharmacopeia (USP), is designed for pharmacy directors, risk managers, patient safety officers, medication safety officers, and other healthcare professionals seeking to enhance their ability to collect, analyze, and prioritize adverse drug event data.

Participants will learn how to select effective risk reduction strategies based on proven medication safety principles, instead of relying solely on human vigilance. They also will learn the best way to report their findings in an actionable format that will help drive medication safety efforts and show results from system improvements.

The workshops will provide take-home materials and tools necessary to analyze and prioritize medication error information and breakout sessions that offer the opportunity to gain hands-on practice working with data.

Reducing Medication Safety Risks: Closing the Gap with the ISMP Self Assessment for Automated Dispensing Cabinets
Join ISMP and its speakers as they present preliminary aggregate findings from data submitted since the launch of the first ISMP Medication Safety Self Assessment® for Automated Dispensing Cabinets at the American Society of Health-System Pharmacists (ASHP) midyear clinical meeting on December 8, 2009. Discussion will include areas for improvement based on the national data along with ISMP staff sharing from their observations during their visits to hospitals. Recommendations for error-reduction strategies to further improve the safe use of ADCs also will be provided through interactive discussion with participants.

The ADC safety assessment is based on guidelines developed by a national forum of vendors and practitioners with expertise in the safe use of ADCs, convened by ISMP. It is uniquely designed to help the nation’s hospitals evaluate their ADC safety practices, identify opportunities for improvement, and compare their experience with that of hospitals of similar size and demographics. The tool is available for free download at www.ismp.org and results can be entered online.
