Learning Objectives:

Following completion of this workshop, participants will be able to:

1. Perform system-based medication error analysis.
2. Identify latent and active failures that lead to medication error events.
3. Develop error-reduction strategies for the use of high-alert medications.
4. Utilize various data collection methods and tools to identify medication-related risk.
5. Discuss the role and responsibilities of the medication safety officer or individual(s) responsible for medication safety oversight.
6. Identify human error, at-risk behavior, and reckless behaviors.
7. Describe appropriate organizational responses to behavioral choices.
8. Measure the effectiveness of selected error-reduction strategies.
