An international medication safety call to action is aimed at preventing fatalities due to medication errors with concentrated potassium injection, inadvertent intraspinal injection of vincristine, and accidental daily instead of weekly dosing of methotrexate in patients with psoriasis or rheumatoid arthritis.

ADCs currently are being used for medication distribution in the majority of U.S. hospitals and health systems, and ISMP believes healthcare organizations have an obligation to ensure that this technology is being optimized to promote safe patient outcomes.
In the wake of recent criminal charges being filed against a registered nurse in Tennessee who was involved in a fatal medication error, ISMP is emphasizing that the focus should be on identifying and fixing imperfect systems instead of unjustly blaming the individual healthcare practitioner.
